    • Not liable for your loans, just a list of people we can reach out to in case we aren’t able to reach you
  • Student Title IV Authorization Form
    • This is a form letting us know how you would like us to use your Title IV funding and any credit balance you may incur
  • Title IV Late Disbursement Form
    • In the event that your Title IV loans don’t disburse prior to your last date of attendance, and you are still eligible to receive, this form authorizes if you want these funds to still disburse
  • Arbitration Agreement
    • Feel free to read through this form on your own and sign at the bottom

Master Promissory Note and Entrance Counseling

For students taking out Title IV, it is required for you to complete the following 2 items:

1) Master Promissory Note (MPN):

  • this is your promise to pay back any loans you take out while going to school.

2)Entrance Counseling:

  • This is your rights and responsibilities as a student
